This training aims to provide participants with the essential skills to investigate workplace incidents thoroughly and effectively. From creating concise incident/problem statements to gathering and assessing critical facts, you will learn how to conduct systematic Root Cause Analysis (RCA) using the 5 Whys method. By mastering these techniques, you'll be equipped to identify the underlying causes of incidents and implement SMART actions that prevent their recurrence.
